If you call after hours or on holidays, your call will be routed directly to our answering service which will help address your needs.
Treatment of Minors
Patients under the age of 18 years old must be accompanied by a parent or legal guardian. Legal guardians must bring written proof of guardianship.
Canceling Appointments
Patients are seen by appointment. Please extend the courtesy to cancel or reschedule appointments at least 24 hours in advance.
Test Results
We strive to offer the best possible care with the help of the most efficient technology available. Our computerized system allows patients to retrieve their test results conveniently and confidentially. If you had a biopsy or another test performed, before leaving the office you will receive a piece of paper with instructions on how to call and receive your test results. Please allow two weeks, before checking on your results. If after two weeks, you call and receive a message that the results are not ready, then please call:

Medical Record Requests
To have a copy of your medical records sent to another physician, you must send us written authorization. Click on this medical request to print the form to complete and then mail it to us or drop it by one of our locations. Patients 18 years or older must sign for their own records. You must supply us with the name and address of the physician who will receive them. Please allow 24 hours for completion of this request.
Form Requests
If you need any medical forms completed such as cancer policies, please call 1-888-337-6722 extension 3870. You may need to schedule an appointment with one of our providers. Please understand that we must adhere to HIPPA guidelines established by the Federal Government. There may be a processing fee which will not be covered by insurance.
Emergencies
If you have a medical emergency regarding your dermatology treatment, such as a surgery patient who is experiencing problems or a patient who is having a severe allergic reaction to a medication, please call our office immediately and dial 0. In the event that you cannot reach our office, proceed to your nearest emergency center.
